Charting on Shifts
🕓 Clock-Out Policy: Documentation Requirements
When your shift ends, Facilities requires providers to confirm their documentation status before clocking out. This ensures your payment and supports quality care.
Below are the three options you’ll see at clock-out — read carefully before selecting:
✅ Option 1: “I completed documentation and charting.”
- Your shift will be automatically approved if all other criteria (e.g., Authorized supervisor signature, geo-clock, break status) are complete.
- Payment will be released without delay.
- No further action needed.
🚫 Option 2: “I did NOT document but was PROHIBITED from doing so”
(Example: you weren’t given login access, the system was down, or you were denied access)
- Your shift will still be **approved **if all other criteria are complete. and get paid.
- You’ll immediately receive a follow-up message requesting you return to complete the documentation the next day.
- The facility will be alerted to verify if you were truly prohibited from documenting.
If it's found that you had access but didn’t document:
- You will be blocked from picking up more shifts until documentation is complete.
- Dishonest claims may lead to removal from the platform.
⚠️ Option 3: “I did NOT document and was NOT prohibited from doing so”
- Your shift will be flagged for manual review.
- No payment will be released until you return and complete documentation.
- The facility must confirm completion before approving the shift.
